TRT UK cost: how much will you really pay?
What a monthly treatment cost doesn't include

Medically reviewed by Dr Chris Philbey, Consultant Endocrinologist at Kliniken

Feeling knackered, foggy and not yourself? It could be low testosterone and here’s what treatment really costs in the UK and what you should get for the money.
You receive less than you’d think for the monthly treatment . But in reality, it’s more than you’d think for what it leaves out.

Trying to find a way to feel yourself again, if you ever typed into Google what TRT is, what the process entails, and TRT cost UK in the night, then you are not hormone shopping anymore.

  • You’re probably flagging by mid afternoon.
  • Your sex drive has gone quiet lately
  • The gym isn’t giving back the results for the effort you are putting in
  • And someone, maybe your partner, maybe you, has started using the word “grumpy” to describe your mood most of the time.

You probably have these questions:

  • How much is TRT in the UK, really?
  • Can I just get it on the NHS?
  • Once I start, is it for life?
  • Will it stop me having kids?

Subscription plans else where promise you above and beyond, but they miss out a lot of crucial aspects, which we won’t shy away from letting you know here.

Tired, flat, not yourself: low testosterone or just getting older?

We have noticed that most men when they reach a point to learn about testosterone and its effects are not trying to learn about TRT specifically, but about why they are feeling off.

These are the symptoms of low testosterone in men we hear about most:

  • A low sex drive and erections that aren’t what they were before
  • Tired all day with even after a full night’s sleep
  • Brain fog and poor concentration
  • A short fuse, low mood or just feeling flat
  • Losing muscle and gaining belly fat
  • Gym progress stalling
  • Poor sleep

If you relate to 3 or more of the above, it’s probably a good time to get your testosterone levels tested.

But there’s a catch to this. The issues mentioned above can also come from being stressed out, having poor sleep, sleep apnea, a thyroid problem, flickering moods, alcohol intake, and even certain medications. Sometimes low testosterone could also be the result of excessive weight gain or an issue with the pituitary gland.

But really, to work out why it’s low before anyone can even decide how to treat it, a number on your blood test is the start.

Did you ever think that snoring heavily and still waking up feeling exhausted could be one of the reasons for low testosterone? In some cases sleep apnea can also lower testosterone and treating it sometimes brings the levels back up on its own. In our consultation do mention it if that is something you feel.

“Your levels are normal”: why so many men get turned away by the NHS

This is the most common story that we usually encounter: a man feels awful, he gets a blood test down from his GP, and he is told that he is totally fine.

The problem is, on paper, he often appears normal because the normal range for testosterone is quite wide. Below 8 nmol/L is when treatment is usually undertaken. Above 12, he is considered to be normal. In between it’s a grey zone where plenty of men feel awful about their condition, but don’t qualify for TRT on the NHS.

But this is not something that can be decided with only one test. Testosterone usually peaks in the morning and as the day progresses it starts dropping. An ideal check would be two early morning blood tests preferably before 11 am.

This makes us look past just total testosterone to free testosterone and SHBG. SHBG is the protein that holds the testosterone in your body which your body is not able to use. Someone with high SHBG can have a “normal” total and still be short of the testosterone that actually does the heavy lifting.

If by any chance you’ve been told that this is normal, yet you’re not feeling it inside, then you’re not imagining it anymore. You may just need someone to look at it more closely.

Had blood test done recently? Bring the results. If they’re recent and complete, we may be able to use them.

TRT UK cost: what you’ll actually pay

Whichever route you plan to take, the TRT UK cost is made up of the same four parts:

  1. A consultation to assess your symptoms and history
  2. Blood tests to confirm low testosterone and check it’s safe to treat
  3. The treatment itself
  4. Ongoing monitoring bloods to keep your dose right and catch problems early

How you pay for those depends on where you go.

On the NHS: If you meet the criteria of the NHS and you fall in the zone that is below 8 nmol/L, then you would pay the standard prescription charge in the UK (which is currently, on average, around £9.90 per item). But the hard part is getting through to that door. Many men are turned away because they fall in the grey zone, as we had mentioned above, and if they’re referred to an endocrinologist the wait times are high.

Online subscriptions: If you’re searching for cheap TRT UK plans, you will usually find monthly subscriptions that start from £60 to £100 a month with some of them reaching £170 a month. Some include monitoring blood tests but others charge for them separately, along with needles and the injection supplies. Most of them have the medication sent to you and you inject yourself at home, often 1 to 3 times a week.

At Kliniken:

  • Your first consultation is with Dr Chris Philbey, a consultant endocrinologist at Kliniken.
  • Blood tests are done here in the clinic to assess your current condition.
  • Your treatment and blood test package is priced at your consultation once Dr Philbey knows what you need.
  • You’ll know the full private TRT cost before you commit to anything.

Whatever TRT UK cost you’re quoted, even from a monthly subscription, make sure you ask what the price includes. Ensure blood test follow-ups and someone to speak to are within the subscription if you feel something is wrong.

Subscription vs consultant led: what the monthly price leaves out

A subscription tries to sell you testosterone based on your obvious underlying symptoms whereas a consultant can go deeper and understand what exactly is causing you to feel knackered. That’s the difference between a subscription and consultant-led care.

The majority of the subscriptions offer you an online video consultation or a phone review with a doctor and then send you medication to inject yourself in the post. Plenty of these doctors know TRT pretty well but with consultant-led care, what you usually get is your own endocrinologist sitting across from you and working on why your testosterone is low in the first place. An endocrinologist can look into places far beyond the obvious symptoms and give you an honest analysis of why you feel low.

At our clinic in Harrogate you will have a direct sit-down with Dr. Chris Philbey, who is a consultant endocrinologist on the GMC Specialist Register. Hormones and the health of hormones are his specialism and he also works as a consultant endocrinologist in the NHS in Harrogate.

Here’s what having Dr. Philbey changes for you:

  • Someone looks for the cause. Thyroid, pituitary, SHBG, weight, sleep and medications all get checked. If something else is behind your symptoms, he’ll find it rather than mask it.
  • Borderline results get a proper look. If you’re in the grey zone, he’ll work out whether your symptoms could be down to testosterone and may offer a treatment trial.
  • Your dose is set for you, not copied from a fixed protocol.
  • Fertility is planned before you start, not discovered afterwards.
  • Injections are given here by our nurses. Nothing is posted to your door.
  • You’re monitored properly, with bloods checked at every review.
  • One consultant from start to finish. No call centre, no rotating prescriber and no handover.

In Dr Philbey’s words: “For those with borderline results, we establish whether the symptoms could be down to testosterone and offer a treatment trial.”

Kliniken is the only consultant endocrinologist-led and evidence based private TRT clinic in North Yorkshire.

Planning on having kids?
Read this before starting TRT

Monthly subscriptions rarely bring this up and this matters more than you think if you are planning to start a family in the future. When you take testosterone from outside the body, your brain notices this and stops its own production of testosterone. The result includes a dropping of the signals that your testes need to make sperm. For many men on TRT sperm count drops sharply and sometimes to even zero.

But don’t worry, it’s something that recovers after stopping although it might take some time.

If you are planning to have kids now or in the near future, feel free to tell Dr. Philbey at your first consultation. Some treatments raise testosterone while protecting fertility raising signals such as hCG, clomiphene, and enclomiphene. Once again this all depends upon your blood work and the plan that you will be put across to.

If you are planning to have a baby in the next year or two, please mention it to us in the beginning. It would change the plan.

Is TRT for life? Can you stop?

TRT could be for life sometimes, but it’s not always the case.

If your body has completely stopped making testosterone on its own, then replacing that testosterone is usually a long-term endeavour.

If your levels have dropped because of something fixable, like we mentioned above about weight, sleep apnea, or some medication you’re on, treating that cause may mean that you don’t need TRT forever.

Do not stop it completely. While you are on TRT your body’s own production slows down. If you come off it suddenly, you might feel even worse than before for a while. If and when it’s the right time for you to come off, Dr. Chris will plan your coming off step by step. This is the difference consultant-led TRT care makes, which a monthly subscription won’t be able to provide.

Side effects: the honest answers

  • Hair loss. TRT can speed up male pattern baldness if it already runs in your family. It doesn’t usually cause it out of nowhere.
  • Your heart. This used to be the big worry. A large study published in 2023 found no rise in heart attacks or strokes in men on TRT compared with a placebo, when treatment was properly monitored.
  • Your prostate. TRT hasn’t been shown to cause prostate cancer. It can raise PSA, so we check yours before you start and at your reviews.
  • Thicker blood. Testosterone can raise your red blood cell count. This is the main thing monitoring catches and it’s usually managed by adjusting your dose.
  • Acne, oily skin and water retention. These can happen in the early months. They’re usually mild and manageable.

As mentioned before these are the side effects and the honest analysis of what you could expect from your underlying condition that a monthly subscription won’t be able to provide. Consultant-led care, if you are worried about side effects, holds your hand step by step with the fears you have. Dr. Chris will address all of them and put you in a state of mind where you would be able to take the call and monitor your progress with TRT.

So is TRT worth it?

  • Probably yes if your blood test and report show that you have low testosterone and this has been analyzed by a consultant endocrinologist after ruling out all other causes.
  • Maybe not yet if you’ve had only one test but nobody has looked for the causes beyond it and you’re planning a family soon.
  • Definitely not if someone is planning to prescribe you a monthly subscription without having a sit-down with you and actually asking you about your history or answering your concerns.

When TRT is the right treatment, most men notice a change in sex drive, mood and energy within a few weeks. Muscle, fat and strength take longer, usually three to six months.

Frequently asked questions

  • How much is TRT in the UK?
The TRT UK cost depends on the route. On the NHS, you pay the prescription charge if you qualify. Online subscriptions charge a monthly fee, usually from around £60 to £170. At Kliniken, your first consultation is £300 and your treatment package is priced once Dr Philbey knows what you need.
  • Can I get TRT on the NHS?
Yes, if your levels are clearly low on repeated morning tests and you have symptoms. Many men in the borderline range don’t qualify.
  • Do I need a GP referral?
No. You can book with us directly.
  • How long does TRT take to work?
Sex drive, mood & energy often improve within 3 to 6 weeks. Muscle, fat & strength usually take 3 to 6 months.
  • Will TRT make me infertile?
It can lower your sperm count while you’re on it, sometimes to zero. If you want children, tell us first. There are fertility-friendly options.
  • Does TRT cause hair loss?
It can speed up hair loss if male pattern baldness runs in your family.
  • Is TRT safe for my heart?
Recent large studies are reassuring when TRT is properly prescribed & monitored.
  • Is TRT for life?
Often long term, but not always. If your low levels have a fixable cause, you may not need it forever. Never stop suddenly without a plan.
  • Can I get TRT without a blood test?
No legitimate prescriber should offer it. If someone does, walk away.
  • Gel or injections?
Both work. Injections mean no daily routine and no risk of passing gel to a partner or child through skin contact. At Kliniken, injections are given here by our nurses.
  • Is TRT the same as steroids?
Testosterone is a steroid hormone, but TRT uses replacement doses to bring you back to normal, not bodybuilding doses.
  • Is TRT legal in the UK?
    Yes, with a prescription.
