What cubital tunnel syndrome actually is

Compression of the ulnar nerve due to a constriction at the cubital tunnel on the inner aspect of the elbow causes this syndrome. It occurs because this area of the elbow is relatively exposed and has very few soft tissues overlying the nerve. Therefore, repetitive activities such as leaning on the elbows while working or repetitive flexion and extension of the elbow cause irritation of the nerve.

Compression of the ulnar nerve results in altered function of sensations to the ring and pinky fingers, as well as motor function of nearly all intrinsic muscles of the hand. Symptoms begin as numbing that occur intermittently. If left untreated, it may progress to weakness and eventual atrophy of affected hand muscles.

Cubital tunnel syndrome is the second most common form of peripheral neuropathy related to entrapment of an upper limb nerve, after carpal tunnel syndrome. This type of injury affects men more frequently than women. Usually, symptoms appear gradually, rather than immediately.

How is cubital tunnel syndrome treatment in Harrogate performed?

Before the procedure

Simple decompression surgery

A small incision is made on the inside of the elbow, and the tight tissue forming the roof of the cubital tunnel is divided. The pressure comes off the nerve immediately. The nerve stays in its original position.

Anterior transposition surgery

This involves moving the ulnar nerve to the front of the elbow. This technique is employed when the ulnar nerve is unstable and repeatedly snaps across the bone during movement or the first surgery hasnt worked.

Medial epicondylectomy surgery

This surgery removes part of the bony bump on the inside of the elbow to widen the tunnel. It is less common but useful in specific anatomical cases.

Most procedures take 30 to 45 minutes. Transposition can take up to an hour.

Aftercare & follow up

You will generally return home on the same day as the surgery. You will have a dressing covering the site of the wound and often times an arm sling. You will need someone to provide transportation after discharge. You will receive prescriptions for pain medication.

What recovery actually looks like after cubital tunnely syndrome treatment?

Recovery is slow. The expectations for how quickly you will recover should reflect that. Once the compression is relieved by surgery, it takes time for the nerve to repair itself. Nerve repairs take a long time because nerve tissue takes a long time to repair.

  • The first couple of days post-surgery are primarily focused on resting the hand, maintaining the wound in a dry state and controlling swelling. Early light finger movements are recommended to avoid having your hand become too stiff. Your sutures generally come out between 10 to 14 days post surgery.
  • By weeks 2-4 most of our desk based patients are able to return to their jobs. Patients are permitted to resume driving as well. In general lifting remains restricted until at least 6 weeks post surgery.
  • After week 4 of recovery, focus shifts towards improving range of motion and increasing grip strength. Hand physiotherapy is often beneficial for some of our patients who develop stiffness limiting function.
  • Most individuals have returned to functional use of their arm within 3 months of surgery and most scars have also matured.
    However, full resolution of numbness and tingling can take significantly longer.
  • Generally, full nerve recovery after surgery can occur anywhere from six to eighteen months after surgery.

Muscle wasting prior to surgery affects the rate of return of strength. Therefore, many times there will be an incomplete recovery of muscle mass lost preoperatively.

This is one reason why early detection of nerve injury is so important.

You feel nothing during the procedure under anaesthetic. Immediately following surgery, there’s swelling around the elbow for approximately one week. Standard pain relievers effectively manage this. Sensitivity within the scars may remain present for two months, however gently massaging them can help resolve the issue over time.

In some mild cases, symptoms will completely disappear by simply making a few lifestyle adjustments, such as wearing an elbow brace at night, limiting hard activities. In other instances, where there has been significant compression to nerves and these same strategies have failed, leaving the issue unaddressed may lead to ongoing degeneration of those specific hand muscles. Although surgery at this later time point will likely continue to slow down muscle degeneration in this area, it does not always restore lost function. The earlier you address this issue, the better your chances for success.
