If you have puffed up, wrinkled or darkened skin around your eyes regardless of how well you sleep, you are probably aware that using lotions and makeup will only go so far. For most people who experience this condition, lower eyelid blepharoplasty (eye bag removal) represents a surgical solution to address the underlying structure that causes these signs. At Kliniken in Harrogate a consultant plastic surgeon or occulo plastics eye surgeon will perform the surgery, typically under sedation or local anaesthetic.
The structural cause of puffiness under the eyes is usually a small pocket of fatty tissue that has pushed through the orbital septum. The soft tissue support around the eye weakens over time. Removing or repositioning this fat creates a smoother contour from the lower lid to the cheek. Excess skin may also need to be removed or tightened during the procedure.

The purpose of this procedure isn't to alter the appearance of your eyes.
Rather it's to eliminate or reduce the swelling and shadow that creates a tired appearance even if you're getting plenty of rest.

As swelling and shadows disappear under the eye, so does the 'bagging' that made you appear as though you hadn't rested well. Now you simply look refreshed and rested.
Once you have fat removed from beneath your eyelids, there is no need to have it removed again. While all of us age and our skin continues to sag over time, most people find results last ten to fifteen years.
By either removing fat from beneath the eye or by placing some of the fat that was taken away from above the bone in the tear trough area, you may experience a softer transition between your cheek and lower lid.
It's possible to have tired eyes and not need eyelid surgery. Many times, fillers and good skincare will help address concerns that people think are structural issues. Blepharoplasty addresses an actual anatomical issue. Surgery will be successful if your concern aligns with what it can correct.

Lower eyelid blepharoplasty is a safe procedure when performed by an experienced consultant plastic surgeon, but no surgery is risk free. The complications below are uncommon and most that do happen are temporary.
You may have noticed some degree of watery eyes for a short period post op. Typically this resolves itself. During consultation we check for existing dry eye prior to surgery, which can be exacerbated by surgery in some cases.
Due to the effect of gravity the lid becomes slightly everted and more white of the eye shows. By employing the transconjunctival approach where possible and by incorporating lateral lid support into the procedure we have greatly diminished the likelihood of ectropion developing.
Following surgery your upper lid sits lower and more of the sclera is exposed. Swelling usually resolves this issue. However in persistent cases, it can often be corrected with a minor revision procedure.
We are all born with asymmetry in general. Any issues related to wound and scar settling are typically resolved within three to six months. If there is any long term asymmetry we will perform a revision once all healing has taken place.
All scars resulting from transconjunctival procedures are completely invisible. Incisions made subciliary rest directly under the lash line and fade to a light pale line over a time frame of six to twelve months. Patients who smoke and those with darker skin tones generally take longer to recover. We review all aspects of scarring during your consultation.
There is a small percentage of patients who require additional surgery to refine their results. This could include addressing any asymmetry remaining, excessive fullness or a small section of skin that didn't contract as desired. Our policy regarding revisions is discussed with each patient during consultation.

This price includes mr Kolar’s surgeon fee, the fee for your consultant anaesthetist, the theatre fees, your choice of either sedation or general anaesthesia (for the safety of your eyes), all post operative follow up appointments and after care products. The vast majority of patients return to their work place and social events at approximately 2 weeks. Swelling and bruising reach their peak approximately 3 days into the recovery period and diminish gradually thereafter. Patients are allowed to resume wearing eye make up after 2 weeks. You may begin to engage in strenuous exercise once they have recovered sufficiently usually by 4 to 6 weeks. Patients typically achieve a complete settlement of their final results within 3 to 6 months.
No. During the procedure you will be placed under sedation or general anaesthesia therefore you will experience no sensation of discomfort during the procedure itself. Many of Mr Kolar’s patients report experiencing tenderness rather than pain after the procedure. Paracetamol is sufficient for managing such discomfort. As a rule, sharp pain following blepharoplasty is unusual and should be brought to Mr Kolar’s attention if experienced.
Fat excised during blepharoplasty remains permanently removed. However, most patients retain a rejuvenated appearance for approximately ten to fifteen years prior to natural aging of the surrounding skin causing further sagging of the lower lids and puffiness to return. The surgery does not stop time but it does reset the clock on both under eye bagging and lower lid puffiness.
No. The purpose of blepharoplasty is to remove structural puffiness and shadow from under the eye. It does not alter your eye shape. Most patients describe their result as simply appearing “rested” rather than “different”, which is precisely the desired outcome.
Depending upon the method chosen by mr Kolar, either none or minimal scarring will occur. Transconjunctival incisions create a small internal opening behind the lower eyelid, leaving no external scar. Subciliary incisions are made along the lash line and will eventually resolve as a faint paler line within six to twelve months. Mr Kolar selects the appropriate method depending on the amount of puffiness or looseness you require correction for and not solely based on speed.
Transconjunctival blepharoplasty involves creating an internal opening behind the lower eyelid, ideally suited for cases involving excessive fat accumulation with little or no skin redundancy. Subciliary blepharoplasty requires making an external incision directly beneath the lash line, required when considerable skin redundancy exists in addition to fat accumulation. In some cases, both methods are used simultaneously.

Due to the precise nature of the operation and risk of involuntary movement during the operation affecting the quality of the outcome, a sedative or general anaesthetic provides the best possible environment for you to remain perfectly still throughout the operation.
You may be suitable for lower eyelid blepharoplasty if you have persistent under eye bags that do not disappear regardless of rest or treatment using fillers or skincare, significant under eye shadows that fail to respond to these treatments or excessive looseness in the lower lids. Only if your dark circles are due to the shadow produced by bulging fat pads. By removing this shadow, blepharoplasty resolves this specific problem. However, blepharoplasty does not correct dark circles resulting from pigmentation or clearly visible veins and arteries underneath thin skin. Mr Kolar assesses the reason for your dark circles during your consultation.
Yes. Many patients elect to perform both procedures simultaneously as doing so reduces overall cost compared to performing each separately as well as having only one recovery period instead of two. An additional consideration is that recovering from combined procedures is generally more uncomfortable than undergoing either individually. Hence, whether to proceed simultaneously is determined jointly between you and Mr Kolar at your initial consultation.
Most patients do not require revision surgery. A very small percentage of patients may require minor revisions to correct residual fullness, slight asymmetry or incomplete tightening of skin remaining.
