What is a sebaceous cyst?

Sebaceous cysts are a type of skin lump which are formed by the blockage of either a hair follicle (a hair’s root) or a sebaceous gland (produces oil). When this occurs, keratin, an oily substance produced naturally in the skin begins to build up underneath the skin creating a pocket or small sac. Sebaceous cysts generally tend to be firm to the touch or slightly rubbery and they typically move under the skin and have a small opening in the center known as a punctum.

Sebaceous cysts are commonly found on the scalp, face, neck, back and/or behind the ears. Typically, these types of cysts are non cancerous (benign), however that does not mean that you should ignore them. Over time, they can get caught on your clothing, cause discomfort, appear more noticeable or become inflamed after being squeezed or drained. They do not usually disappear on their own. What does sebaceous cyst removal involve?

Sebaceous cyst removal surgery at Kliniken Harrogate is typically done as an outpatient procedure using local anaesthesia.
Our goal is to remove both the cyst and the capsule so that we can minimize the risk of the cyst coming back.

Consultation and assessment

Initial assessment includes an examination of the lump, confirmation as to whether the lump is a sebaceous cyst, discussion of the size and location of the lump and explanation of the procedure for removal of the lump, discussion of the likely appearance of the scar and how long your recovery will take and whether it is appropriate to send your lump for histological examination.

Numbing (Local anaesthesia)

Next, the area is completely numbed using local anaesthesia. The injection of the local anaesthetic may cause a stinging sensation for a short period and for many people, this is the most uncomfortable part of the whole procedure. When the area has been sufficiently numbed, you may experience some pressure or movement but you should not feel any pain.

Removal of complete cyst capsule

An incision is made over the top of the cyst and the surgeon carefully removes the cyst and the entire capsule. It is generally considered to be the capsule that is most important to remove in order to prevent recurrence of the cyst. The incision is then closed. The procedure usually takes approximately 20-40 minutes.

Recovery & aftercare

Day 1

The area of the body where you had surgery will probably be painful and a little tender once the local anaesthetics wear off. Mild swelling and bruising near the incision are common side effects. However, many patients find they can take OTC pain medication like paracetamol. It's also recommended to just relax for the remainder of the day.

Days 2 – 3

Generally, by this time, the discomfort will have subsided significantly. Desk work and normal light activity are usually okay as long as the wound is protected from undue stress and the patient keeps the area clean and dry if so directed by their surgeon.

Days 7-14

Patients typically return to the clinic for suture removal within this timeframe. By this time, the wound should be starting to heal properly and should feel much better. Patients whose cyst was located in areas of the body that tend to move a great deal like the upper back or shoulders, may experience some tightening in the skin for an extended amount of time.

Weeks 2-4

During this timeframe, most patients can begin to return to a regular exercise program and other physical activities. The timeframe for returning to these activities will depend upon several factors including the size of the cyst, the location of the incision, and the amount of stress the area experiences due to movement in the patient's daily routine.

The Scar

There will always be some degree of scarring associated with any surgical excision of a cyst. Initially, the scar will likely appear red and raised. As the scar matures over the next three to six months, it will flatten out and lighten in colour. Cysts located in the scalp and face often heal very nicely, however, the quality of the resulting scar varies greatly among individuals.

Risks and what to expect honestly

You should have some idea about what might happen and what is normal when you undergo sebaceous cyst removal in Harrogate. Like every surgery, there are risks involved with sebaceous cyst removal, but they are generally minimal.

Surgical technique plays a role here. Histological confirmation of the diagnosis may be performed after removal, but it is the complete excision of the cyst that reduces the likelihood of recurrence. Sebaceous cysts and lipomas are two different types of lumps. Once settled, removal of the cyst can take place after the inflammation subsides.
