What is a fleur-de-lis tummy tuck?

The primary difference between a standard tummy tuck (abdominoplasty) and a fleur de lis tummy tuck (abdominolift) is how the additional incision helps the surgeon remove the hanging skin on either side of the abdomen. The horizontal incision on a standard tummy tuck allows removal of the hanging skin on the front of the abdomen.

The addition of the vertical incision gives the surgeon access to tighten the muscle layers and remove the hanging skin on the sides of the abdomen. This is particularly useful after significant weight loss where there has been significant stretching of the abdominal wall tissue on all sides, not just on the front.

Standard, extended or fleur-de-lis: which do you need

Whether you need a standard abdominoplasty, extended abdominoplasty or fleur de lis abdominoplasty depends upon where your excess skin is located. Abdominoplasties can remove skin from side to side or from the lower abdomen. Depending on the location of your excess skin, there are three types of procedures.

Standard tummy tuck

This procedure removes excess skin from the lower abdomen, creating a longer incision horizontally across the abdomen. The best candidate for this surgery would be someone whose excess skin is primarily located at the bottom of their abdomen.

Extended tummy tuck

The extended version removes extra skin from the side of the body, and extends up towards the hip. Therefore, if your skin sags toward the sides, then this could be the better choice.

Fleur-de-lis Abdominoplasty

Removes excess skin from both sides downward. If your skin has sagged significantly due to extreme weight loss, this could be the best method for you.

NOTE:  There is a downside to this procedure of scarring. A standard and extended abdominoplasty leaves a single horizontal scar while a Fleur De Lis abdominoplasty includes a vertical scar going up the center of the stomach.

However, by removing much more skin than the previous two options, it is a fair trade off for some patients.

Am I a candidate for a fleur-de-lis tummy tuck

People with loose skin hanging horizontally and vertically in front of them are candidates for a fleur de lis tummy tuck. Specifically:

You have recently lost a lot of weight either by dieting, exercising or undergoing weight loss surgery.

In some cases, loose skin after pregnancy can also benefit from a fleur de lis tummy tuck. However, most individuals who require the use of a vertical cut have experienced major weight loss. Both men and women who experience significant weight loss are equally likely to be suitable candidates for this form of tummy tuck.

How does recovery after fleur-de-lis surgery look like?

  • Many patients require 4-6 weeks away from their jobs and longer time may be needed depending upon the nature of their employment.
  • The first two weeks after surgery can be difficult due to feeling tighter than normal across your abdomen and needing to walk slowly and bend slightly as you recover.
  • You will also wear a compression garment continuously for approximately six weeks.
  • Light walking can begin immediately to decrease the chance of developing a blood clot.
  • Typically, driving begins again by 4 weeks post-op and regular exercise by six weeks.

What are the risks associated with this surgery?

Each surgical procedure has associated risks which you should consider prior to undergoing the procedure.

  • A fleur-de-lis tummy tuck contains all the risks associated with any type of tummy tuck. These include: bleeding, infection, seroma (the accumulation of fluid beneath the skin), loss of sensation in the skin, delayed wound healing and blood clots.
  • An additional risk is present with the vertical incision. This would be the junction of the two incisions located near the bottom of your abdomen. This location is under significant tension therefore this area is at increased risk for delayed wound healing. However, careful closure and adherence to proper wound care will minimize this risk.

✦ How is fleur-de-lis abdominoplasty done

How the procedure works

A typical fleur de lis tummy tuck procedure can take anywhere from three to four hours under a general anesthetic.

Step 1. Making Incisions

Two incisions will be made. One cut will be made horizontally at the bottom of the stomach and another vertically along the mid-line. These two incisions will form a reverse-T shape. Depending on how much extra skin that needs to be removed, the size of these incision will vary.

Step 2. Repairing the Muscles

After lifting all of the excess skin off of your body, your surgeon will check for diastasis recti, which occurs when your abdominal muscles separate. If this has occurred, the muscles are then sutured or sewn back together and this provides you with a flat appearance inside as well as out.

Step 3. Removing Excess Skin

All remaining extra skin is then removed from both sides; the top and bottom. Your surgeon then pulls the remaining skin together to fit over the tighter muscle mass and repositions your belly button so it sits in its natural position.

Step 4. Closing Up & Drains

Each incision is then closed using sutures that dissolve underneath the skin so there won’t be any need to come in for them to be removed. Some surgeons may also choose to use drainage tubes after surgery. These tubes are usually removed before your first post-operative visit.

✦ An important caveat to be noted

The vertical scar and how it heals

A vertical scar is the honest trade off made for the ability to remove skin that an average tummy tuck can't access.
The trade off is a line that goes vertically through your abdomen and a horizontal line at the bottom.

  1. At first the scar will be raised and red. After 6 to 12 months the scar will flatten out and fade into a lighter colour.
  2. Since the vertical scar will run directly down the center, most of it will be covered by most underwear and swimwear waist bands.
  3. However, depending on the style of your bikinis, some of the top portion of the scar may be visible.
  4. Our doctor will place the incision along the normal midline and close it in multiple layers to make it as smooth as possible.
  5. We will give you instructions on caring for the scars usually applying silicone gel or using special tapes once the area has healed enough, which will help the scarring process occur sooner.
