What is an infected cyst?

An infected cyst is essentially a benign lump of skin which is typically an epidermal or sebaceous cyst grown as a sac under your skin that has accumulated keratin, a protein produced by your body. Typically people do not think about their cyst until they become symptomatic and it becomes problematic.
Cysts become infected from bacteria getting into them. Bacteria are commonly introduced via a blocked hair follicle, minor trauma (such as picking or popping) or rupture. Infection leads to inflammation, therefore the symptoms you see are due to this response.
Important for you to understand, however, is that antibiotics can be used to treat the bacterial infection but will not remove the cyst which means that the same cyst may become infected again. This is because there was no removal of the sac which is the source of the infection. To prevent recurrent infections, the sac must be completely removed along with the cyst.

Cysts may lay dormant under the surface of the skin for long periods of time without much trouble. If one becomes infected, changes will be readily apparent. Generally there will be more pain than usual and some degree of redness or heat around the area with possibly some swelling that seems to be building. Sometimes, the infected cyst has ruptured enough to allow pus or discharge to appear.


Most often, an infected cyst will require medical attention. The good news is that once treated, the infection typically responds well to proper treatment.

How is an infected cyst removed?

Our goal is to remove both the cyst and the capsule so that we can minimise the risk of the cyst coming back.

Consultation and assessment

The surgeon looks at the cyst and verifies the diagnosis. You will talk about the size and location of the cyst and our surgeone will mention the method used in the procedure, the appearance of the scar and the time that recovery will take. If needed the surgeon will tell you if the lump needs to have a histological examination done.

Numbing (Local anaesthesia)

The area is completely numbed using local anesthesia. The injection may create a brief "stinging" feeling for most people which is usually the most uncomfortable part of the whole procedure. After the area has been properly numbed, you may feel some pressure or movement, but you shouldn't feel any pain.

Removal of complete cyst capsule

A small incision is made above the cyst and the surgeon gently removes the cyst and the entire sac. Infected cysts may leave the wound open instead of closing it right away which is intentional with no complications. Closing an open wound that contains bacteria could seal them into the wound. An open wound allows the tissue to heal by itself from the inside out. The procedure typically takes 20-40 minutes.

You can go back home immediately after the procedure and you will receive written instructions as to your aftercare. The sutures will be taken out at the first post-operative visit, which is usually 7 to 14 days depending on where the cyst was located and how the wound was closed.

Recovery & aftercare

Day 1

A local anaesthesia can cause temporary numbness in the area. The numbness will wear off eventually and when it does, you may experience some mild discomfort. In most cases, taking paracetamol is all you need to alleviate this. Do not get the dressing wet. Avoid touching the area.

Days 2 – 5

Bruising and swelling are common side effects. If your wound was left open as opposed to being stitched which is often the case with infected cysts, the wound may appear more "raw" than you would have anticipated at this point. This is completely acceptable. The wound is healing as it should be.

Days 7-14

You will receive guidance for cleaning the wound site at your appointment. A follow up will be scheduled so we can assess how well the wound has healed and remove sutures if necessary.

Weeks 2-4

Office-based work: Most individuals return to their office jobs the next day or shortly thereafter. Depending on the location of the cyst removed, returning to physical labour will typically take 1-2 weeks.
Returning to Exercise: Light activity: Usually after about a week. Any strain to the wound site: 2-3 weeks.

The Scar

There will always be some degree of scarring associated with any surgical excision of a cyst. Initially, the scar will likely appear red and raised. As the scar matures over the next three to six months, it will flatten out and lighten in colour. Cysts located in the scalp and face often heal very nicely, however, the quality of the resulting scar varies greatly among individuals.

Risks and possible complications

Many people recover perfectly well after infected cyst removal surgery. However, all surgeries carry risks so it’s important to understand them. The most common problems are very minor such as bruising, swelling, pain or discomfort around the site of the wound and occasionally, the wound may take a little longer to heal, especially if you had an open wound after the removal due to infection.

However, the main concern for infections specifically relating to cysts is the possibility of incomplete removal of the cyst. Because the cyst is often inflamed, it adheres to the surrounding tissue making clean removal difficult. This means that there is a chance that fragments of the cyst remain. In this eventuality, the cyst will recur. For this reason, timing of the operation is very important and is also one of the many reasons that we often suggest treating the infection before operating.

Additionally, there is a slight risk of the infection remaining after the excision of the cyst. In this case, further treatment with antibiotics would typically be required.

Other less common risks include post-operative wound infection, a larger than anticipated amount of scarring and temporary loss of sensation around the scar.
